At Fordbridge Medical Centre we seek
to provide the best possible medical
care to our
patients. We encourage you to be
involved in your health care and to
participate in a
partnership with the medical team.
This two-way relationship allows us
to make the best
decisions about your care and
treatment.
Human Rights
We believe that the people who use
our services, their carers and our
staff should be treated
with respect and dignity. This means
we will not tolerate racism,
homophobia, sexism or any
other form of discrimination against
staff, service users or carers.
What You Can Expect From Us
To be treated with courtesy and
confidentiality. To be seen the same day for
conditions you and your Doctor agree
are urgent. To obtain access to a Health Care
Professional within 48 hours. To receive healthcare in safe,
comfortable and appropriate
surroundings. To be seen within 30 minutes of
any appointment you are offered at
the surgery, failing this to be offered an explanation. We will prescribe the most
suitable treatment and will fully
discuss your medication with you. We will try and provide all repeat
prescriptions within two complete
working days of ordering. To have your long term medication
and treatment reviewed at regular
intervals. To be informed (through leaflets,
and our website etc) of the
practices services.
What We Expect From You
Please treat doctors, nurses,
staff and other health professionals
with courtesy and respect
as we operate a zero-tolerance
policy and will not tolerate abusive
or violent behaviour as
these patients are removed from our
lists. Attend appointments on time or
cancel appointments in plenty of
time to enable them to be
given to someone else. To make more than one appointment
if more than one person needs to be
seen. To be prepared to make further
appointments if you have numerous or
complicated
problems. To ask for a home visit only if
housebound or the illness prevents
you from attending
surgery. Children can usually be
brought to the surgery safely. Give 48 hours notice when ordering
repeat prescriptions. Be prepared to have regular
reviews. Be prepared to accept that you do
not always need a prescription. Be patient; do not blame the
receptionists if surgeries are
running late.

Confidentiality
We respect your right to privacy and
keep all your health information
confidential and secure.
It is important that the NHS keeps
accurate and up-to-date records
about your health and
treatment so that those treating you
can give you the best possible
advice and care. This
information is only available to
those involved in your care and you
should never be asked for
personal medical information by
anyone not involved in your care.
You have the right to know what
information we hold about you. If

Under the Data
Protection Act we do not share your
information with any other persons.
